LONDON -- New findings from a recent, international, open-label study reaffirm that the patient-applied treatment, Aldara (imiquimod) Cream, is safe and effective for eradicating external genital warts -- the results of which are reflective of a real-life clinic setting.
The study reports that the efficacy of Aldara in male and female patients of different ethnic backgrounds was similar to that in previous randomized, vehicle-controlled studies, according to the International Journal of STD & AIDS.
This international trial is the first published study that investigates the efficacy of a topical therapy for external genital warts in such a large group of patients -- in 114 clinics across five continents (Europe, Canada, Latin America, Australia and South Africa).

This open-label, dose escalation study was conducted in clinics in Germany and France. Male patients (n=64), with an average age of 30 years, were treated until clearance of baseline target warts or for up to 16 weeks. Aldara was well tolerated, with the two most frequently reported local skin reactions being erythema and erosion. Two thirds of patients had undergone previous treatment for genital warts with various surgical or caustic therapies, which, as the investigators noted, can be inappropriate for uncircumcised males. A topical prescription drug called Aldara completely eradicated genital warts in 80% the people who applied it thrice weekly for three-months, according to a study published in last month's issue of Archives of Dermatology. The participants were healthy men and women with two to 50 external warts in the anal/genital area. Two different doses of Aldara were used in the study, 5% and 1%. The higher strength provided better results. The warts completely disappeared in half the people using the 5%.
Genital warts, wrote the investigators, Libby Edwards, M.D. and colleagues, are "benign tumors induced by any of multiple different types of the human papillomavims." The usual treatments for this condition must be doctor-administered. They are not only painful and drastic (e.g., freezing, cauterizing, excision) but they also have high failure rates (2-72%).
Recurrences occur in Aldara-treated people too, but the investigators say the rate is lower than that of the more drastic treatments. Although this study lasted only 12 weeks, Dr. Edwards and colleagues observed a far lower recurrence rate even among the study participants who were using the weaker 1% version of Aldara. In a telephone interview, Dr. Edwards explained, "It's extremely unlikely that Aldara eradicates the human papillomavirus in most patients." This is true of other treatments, but Dr. Edwards cited a major advantage for this topical medication:all other treatments adversely affect the skin surrounding the warts."
